Rifaximin

Tablet

Rifaximin is a non-absorbable, broad-spectrum antibiotic derived from rifamycin. It acts locally in the gastrointestinal tract, reducing bacterial load and toxin production. Its minimal systemic absorption makes it safe for long-term use in hepatic encephalopathy and IBS-D.

Mechanism

Rifaximin inhibits bacterial RNA synthesis by binding to the beta-subunit of DNA-dependent RNA polymerase. Its action is confined to the gut lumen due to poor systemic absorption, making it effective for gastrointestinal infections and conditions.

Pharmacokinetics

  • Absorption: Minimal systemic absorption
  • Distribution: Localized action in GI tract
  • Metabolism: Hepatic metabolism (minor)
  • Elimination: Primarily fecal excretion
  • Half-life: ≈ 6 hours

Side Effects

  • Nausea
  • Flatulence
  • Abdominal pain
  • Headache
  • Rare: hypersensitivity reactions

Contraindication

  • Hypersensitivity to rifamycin derivatives
  • Severe hepatic impairment (use cautiously)

Interaction

  • Minimal systemic interactions due to poor absorption
  • May interact with P-glycoprotein substrates
  • Monitor with other rifamycin derivatives

Warning and Precautions

  • Not effective for systemic infections
  • Monitor liver function in long-term use
  • Discontinue if severe hypersensitivity occurs
